Silcream 1% Cream acts on the cell wall and cell membrane of the bacteria to cause their death.

Transient leukopenia, skin necrosis, erythema multiform, skin discoloration, burning sensation, rashes, and interstitial nephritis.

Indication of Silcream 1% Cream

Silcream 1% Cream is used for the prevention and treatment of wound infections in patients with second and third-degree burns.

Pharmacology

The mechanism of Silver Sulfadiazine's antibacterial action involves structural changes in bacterial cell membranes, including distortion and enlargement, weakening of the cell wall membrane, and reduced viability in sensitive strains. The silver moiety binds to bacterial cells, attaches to DNA, and prevents bacterial cell proliferation. Sulfadiazine provides bacteriostatic action against sensitive organisms.

Dosage of Silcream 1% Cream

Missed Dose

Use/apply the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose. Do not double your dose to make up for the missed one.

Overdose

An overdose of Silcream 1% Cream is not likely to cause severe symptoms. However, ingestion of this medicine may cause harm and immediate medical intervention may be required.

Administration of Silcream 1% Cream

• Cleanse the burn wounds and apply Silver Sulfadiazine cream over the burn wound.
• Cover the burn areas with the cream at all times.
• Apply once to twice daily to a thickness of approximately 1/16 inches or 1.5 mm.
• Reapply whenever necessary, especially after hydrotherapy.
• Treatment should continue until satisfactory healing occurs or until the burn site is ready for grafting.

Interaction

• Silver sulfadiazine may inactivate enzymatic debriding agents, making concomitant use inappropriate.
• Potentiation of oral hypoglycemic agents and phenytoin action may occur, requiring monitoring of blood levels.
• Co-administration of Cimetidine may increase the incidence of leukopenia in patients with large-area burns.

Contraindications

• Hypersensitivity to Silver Sulfadiazine or any other ingredients in the preparation.
• Use on pregnant women approaching or at term, premature infants, or newborn infants during the first 2 months of life.

Pregnancy & Lactation

Pregnancy Category B. Use during pregnancy only if justified. It is not known whether Silver Sulfadiazine is excreted in human milk.

Precautions & Warnings

• Monitor hepatic and renal functions in patients with impaired elimination.
• The possibility of silver inactivating topical proteolytic enzymes should be noted.
• Monitor serum sulfa concentrations in patients with extensive burn wounds.
• Renal function should be carefully monitored, and urine should be checked for sulfa crystals.
